Submit an application on FACTS
- Current report card (and transcript for transferring students)
- Teacher & Character Recommendation forms
- Immunizations
- Individual Service Plan such as 504 or IEP (if applicable)
- Submit Social Security Card or Birth Certificate
- Complete student interview
- Complete student placement test (cost of $50 paid on test day)
- Be accepted into the Cardinal flock
- Pay enrollment fee

- Excel at home and at school
