To ensure your child is eligible to participate in CHS athletics, please complete the following forms and return to Coach T (tristan.winkelman@chsaustin.org) as PDFs
TAPPS Physical: A current physical examination dated within the past year.
Medical History Form
Signed CHS Athletic Handbook
Team Registration: Complete the Google Form for any sport your athlete plans to participate in.
Student Transfer Form (STF): Required for transfer students.
Previous Athletic Participation Form (PAPF): Required for transfer students.
International Student Form: Required for all international students.
Student Not Living With Parent Form: Required for all students not living with parent.

FOR STUDENTS
Go to tms.tapps.biz on a desktop or download the free TAPPS TMS app.*
New students only:
Enter your school email, click "Forgot Password" to get a one-time password (OTP) email.Use the OTP to log in and set a new password.
Once logged in:
Go to your Profile.
Fill in your Emergency Contact and 1 Guardian (you’ll need their name, email, and phone number).
Answer the 3 questions.
Click on "Acknowledgment of Rules (AOR)" and hit SAVE.
Let your parent/guardian know you’ve finished, so they can do their part next!
Already have an account? Just log in and accept the AOR for the school year.
PARENTS/GUARDIANS – Do This After Your Student
Go to tms.tapps.biz on a desktop or download the TAPPS TMS app.*
Use the same email your student entered for you in their profile.
Click "Forgot Password" to receive a one-time password (OTP) email.
Log in using the OTP, set your new password.
Complete your Profile (if needed).
Click on the "Acknowledgment of Rules (AOR)" and hit SAVE.
Already have an account? Just log in and accept the AOR for each student linked to you.

Athletic Registration
Please provide all information required on the sports sign-up, read and agree to the enrollment acknowledgement, and submit to officially register your son or daughter. Any questions regarding sports sign-ups should be directed to Tristan Winkelman, Athletic Director at Tristan.Winkelman@chsaustin.org.
